Because physical therapy may require prior authorization from your insurance carrier, we ask that you provide us with your insurance information and demographics. You may call us with this information or you may download the New Patient Information Form provided below and fax it to us at (512) 795-0043.

*You do not need to print the Privacy Notice but please read it and bring the acknowledgment form.
